Mediterranean Water Shrew is small, 6 to 9 cm trunk, and 4 to 6 cm tail, with a total weight of between 7.5 to 16 g . The color varies, but the belly is always clear, almost whitish, and the tail is bicolor. The ears are completely hidden in the coat, the snout is thick and the teeth are garnet.

The border of swimming sows in the hind feet is less developed than in other species of musgaño (as for example the white-legged musgayan, Neomys fodiens ), because N. anomalus is less related to water.
