CNDDB draft form

Howdy team, here's a draft of the current (2015) CNDDB form. All spatial information will be taken from the phone GPS so those are excluded. I know this feels field heavy, but it should be a good reproduction with lots of room for explanations. Please let me know what you think. - Lech
